MEDISEP Phase 2 (2026) — What Kerala Hospital Billing, TPA and Front-Office Staff Need to Know
Kerala's health insurance scheme for state government employees and pensioners entered Phase 2 on 1 February 2026 with higher cover, more treatment packages and new rules for non-empanelled hospitals. Every empanelled hospital's admission, billing and insurance desks now work to these rules.
MEDISEP (Medical Insurance for State Employees and Pensioners) is the Government of Kerala's cashless health insurance scheme for state government employees, pensioners, university and local-body employees and pensioners, and their dependents — nearly 32 lakh people. Phase 2 began on 1 February 2026: cover rose from ₹3 lakh to ₹5 lakh a year, treatment packages rose from 1,920 to 2,516, the premium is ₹687 a month, there is no age limit, and Oriental Insurance continues to implement it.

MEDISEP Phase 1 vs Phase 2 at a Glance
Figures as reported when Phase 2 was rolled out in January 2026. Hospitals should always work from the current package list and circulars issued by the Government of Kerala.
Who Is Covered — and What Is Not
MEDISEP covers state government employees, service and family pensioners, employees and pensioners of universities and local self-government institutions, and their dependents. Phase 2 has no age limit, and medical and surgical treatment are covered together.
- Cashless at empanelled hospitals: every empanelled hospital is expected to offer cashless treatment, and new agreements push hospitals to empanel all possible departments.
- Emergencies at non-empanelled hospitals: accidents, heart attacks and strokes are covered even at non-empanelled hospitals, and Phase 2 adds reimbursement for 10 more treatments outside the network.
- Specialty centres: treatment at Sree Chitra Tirunal Institute (SCTIMST) and JIPMER is reimbursable.
- Critical illness: the additional cover for 10 extremely grave conditions and organ transplant surgeries continues.
- Not covered: outpatient (OPD) treatment.
What MEDISEP Phase 2 Changes for Hospital Administration Teams
In an empanelled hospital, MEDISEP is run day to day by the administration side — not the doctors. Phase 2 changes that work in five places:
- Front office and admissions — verifying the beneficiary's MEDISEP ID at admission and explaining what is cashless and what is not (OPD is not).
- Insurance / TPA desk — choosing the right package from the larger 2,516-package list, raising pre-authorisation correctly and following up until approval.
- Billing — applying the new room-rent limits (up to ₹5,000; ₹2,000 in government pay wards) so the patient is not surprised by the difference at discharge.
- Day-care units — using one-time registration for dialysis and chemotherapy patients instead of re-registering every visit.
- Medical records — keeping the discharge summary and investigation reports complete, because claim settlement depends on them.
These are exactly the desks — TPA coordinator, billing executive and front office — where Kerala hospitals hire hospital administration graduates.
